Accidental Death & Dismemberment Insurance (AD&D), issued by The Prudential Insurance Company of America (Prudential), can provide you or your beneficiaries with money if you die or suffer a covered loss due to an accident. Because of this, it’s also commonly called Personal Accident Insurance (PAI).
Who is eligible for this coverage?
All individual NHCLC members under the age of 70 who are either U.S. citizens or permanent residents of the United States are eligible to apply for the NHCLC Accidental Death & Dismemberment Insurance Plan. Coverage amounts (or Principal Sum) up to $500,000 are available so you can select the amount that best protects your loved ones. And if your needs increase, you can apply for more coverage.
How much does coverage cost?*
AD&D coverage is relatively inexpensive, with $50,000 of coverage costing just $3.50 per month. A full rate chart is shown below. Coverage reduces at age 65 and 70 and terminates at age 75. Details are in the chart below.
What does the insurance cover?
The chart below breaks down the percentage of your coverage amount you would receive if you suffer a covered injury.
A member’s coverage reduces 50% at age 65 and another 50% at age 70 to a maximum benefit of $25,000. Coverage terminates at age 75.
*Administrative costs will also be added: Direct Bill are $3.00 per bill, Credit Cards are $2.00 per bill, and Automated Clearing House (ACH) are $1.00 per bill.

Can these rates be increased?
Like most group insurance policies, rates are subject to change, but only on a class-wide basis. You can never be singled out for a rate increase while you are insured under a group policy issued to the NHCLC and provided that you continue to be an active member of the NHCLC.

Provides a daycare benefit for a covered child under age 13 if the member or covered spouse/domestic partner dies in an accident.
Home Alteration and Vehicle Modification Benefit
Provides a one-time benefit if the insured suffers a loss that requires home alteration or vehicle modification.
Child Tuition Reimbursement Benefit
Provides a benefit for a covered child who is under age 26 and a full-time student in an institution of higher learning beyond 12th grade, if the member or covered spouse dies in an accident.
Return of Remains Benefit
Provides a benefit if the insured suffers a loss of life outside of a 100-mile radius of their home.
Spouse Tuition Reimbursement Benefit
Provides a benefit for a covered spouse/domestic partner for professional or trade program if the member dies in an accident.
Monthly Mortgage Payment Benefit
Provides a benefit to help pay the monthly mortgage payment if the insured suffers loss of life.
When the AD&D coverage terminates - Coverage terminates at the earlier of termination of association membership, the premium due date on or next following member’s or spouse’s (if applicable) attainment of age 75, the date in which premiums due are not paid, your written request to terminate coverage, or termination of the master policy.
Exclusions - A Loss is not covered if it results from any of these: 1) Suicide or attempted suicide, while sane or insane. 2) Intentionally self-inflicted Injuries, or any attempt to inflict such Injuries. 3) Sickness. 4) Medical or surgical treatment of Sickness. 5) Any bacterial or viral infection. But, this does not include: a) pyogenic infection resulting from an accidental cut or wound; or b) bacterial infection resulting from accidental ingestion of a contaminated substance. 6) Taking part in any insurrection. 7) War, or any act of war. War means declared or undeclared war, and includes resistance to armed aggression. 8) An accident that occurs while the person is serving on full-time active duty for more than 30 days in any armed forces. But this does not include Reserve or National Guard active duty for training. 9) Commission of or attempt to commit an assault or a felony. 10) Travel or flight in any vehicle used for aerial navigation (includes getting in, out, on or off any such vehicle), if: a) the person is riding as a passenger in any aircraft not intended or licensed for the transportation of passengers; b) the person is performing as a pilot or a crew member of any aircraft; or c) the person is riding as a passenger in an aircraft owned, operated, controlled or leased by or on behalf of the Contract Holder or any of its subsidiaries or affiliates. 11) Being legally intoxicated or under the influence of any narcotic unless administered or consumed on the advice of a Doctor. 12) Participating in these hazardous sports: scuba diving, bungee jumping, skydiving, parachuting, hang gliding, paragliding, paramotoring, parascending or ballooning
This policy provides ACCIDENT insurance only. It does NOT provide basic hospital, basic medical, or major medical insurance as defined by the New York Department of Financial Services.
